ADULT EVALUATION
AND REVIEW SERVICES
The AERS team helps
functionally disabled adults maintain their independence. The AERS
nurse and social worker visit individuals at home and recommend
helpful local services. The AERS team helps patients and families
contact chosen services and assures their effectiveness. Please call
