Birth and Death Certificate
Information
Birth
Certificates are now available at local health departments.
You
may obtain a birth certificate at the Queen Anne's County Department of Health
for an individual born in the State of Maryland
from 1939 to present. A non-refundable $20.00 fee and identification are required.
You may also obtain a birth certificate from the Division of Vital Records. Visit the Division of Vital Records website for more information and applications.

Who Can Obtain a
Certificate?
- Person asking
for his/her own birth certificate.
- The parent or
guardian of the person (a foster care appointment is not
acceptable). Proof of guardianship must be submitted.
- A representative
of the person, parent, or guardian. The representative must
have a letter signed by the person, parent or guardian, and
certified by a notary public saying that the representative has
permission to obtain the birth certificate.
- A court of law.

Procedure for Obtaining Copies in Person

Appear in
person at the Queen Anne's County Department of Health

Birth
records are only available to the persons named on the record
or an authorized representative. Authorized representatives
must submit a court order or a notarized statement signed by a
person named on the record. If authorized to obtain a birth
record, provide the full name at birth, date of birth, place
of birth (County or City), mother's full maiden name, father's
full name, your relationship to the person named on the record
and your name and mailing address.
